Requirements for Application

Necessary Requirements to be presented in securing an Application Form

1.  Original copy of Birth Certificate from the National Statistics Office and two (2) clear     
photocopies 

2.  Progress Report Cards

*Kinder 1 - Certificate of Attendance / Narrative Assessment Report / Progress Report (2 clear photocopies)

*Kinder 2 to Year 2 - previous and present Report Cards (2 clear photocopies)

Note: Please submit authenticated scholastic records. Procedure for Foreign / Dual Citizen and Homeschooled student applicants

3.  Certificate of Recognition from DepED coming from the present school

4.  Baptismal Certificate - present original and one (1) photocopy

5.  Two (2) passport size recent colored photos - cut-outs are not accepted

6.  One (1) long folder

7.  One (1) long brown envelope

NOTE: Application and Testing Fee - Php 600.00 (Non-refundable)



FOR FOREIGN STUDENTS

Certified True Copies of  the following:

  1. Alien Certificate of Registration
  2. Photocopy of Passport
  3. Visa / working permit of parents who are non-Filipino citizens
  4. Certificate of Recognition for Dual Citizenship
  5. A copy of the scholastic records authenticated by the Office of the Philippine Embassy / Consular Official (with the English translation of the applicant's name, grades, and Grade/Yr. level)

Note:  Student-applicants who have very little or no knowledge in the Filipino subject     
should at least take six (6) months to one (1) year of tutorials before enrolling in
Assumption.
